Joanne Van Dyke, Roxanne Dominquez, and Brenda Tormis and their dedicated, hardworking team of nurses, therapists, administrative personnel and nursing assistants provide the best nursing home care in our region.
As one of the Kaiser Permanente Skilled Nursing Facility Rounding Physicians, I am continuously working with the various nursing home staff and facilities in our region. I can say without a doubt that Fairfield Nursing and Rehab is the best of the group.
Specifically, they treat patients and their families with detailed medical professionalism wrapped in love and sincere caring. The medical orders and treatments I write for our always carried out with reliable thoroughness. The staff is vigilant regarding acute changes in my patients and is quick to inform me or one of my staff.
The Therapy staff is top notch. I am continually amazed at how hard they work to get our patients to the highest level function. Their goal is to quickly assess and improve each patient on a daily basis.
At Kaiser Permanente, we rely on the dedicated staff of Fairfield Nursing and Rehab to provide the best care for our patients. They consistently do this and more. I have nothing but high praise for everyone working with and for Joanne and her staff.

Recently, a family member of ours was hospitalized for numerous health issues that prevented her from returning to her home in Fairfield. Not being able to care for her medical needs at our own home, we did not know where to turn until we were referred to Fairfield Nursing and Rehabilitation Center by the hospital social worker.
Since we had no previous experience with nursing facilities, we were very apprehensive and concerned about placing a loved one in their care. However, it turned out to be a real blessing. The admission process was simple and the staff cordial and helpful. From the first day, we knew that we had done the right thing. Medical,
nursing, and service staff personnel immediately began providing expert care, and occupational and physical therapy quickly followed. Everyone in the facility is professional, courteous and friendly. It is a 24-7 operation, and the staff is constantly busy tending to the residents. Every resident is treated with dignity and respect
for his or her privacy. To the administrators and staff in all these facilities: Thank you for your outstanding support to our community.

I would like to thank the Fairfield Rehabilitation Nursing Home for providing exceptional medical care and supportive services to my Mother, Ms. Eunice Bellard. The medical and administrative staff members exemplified the true efforts needed to comfort my mother whom suffered from a multitude of medical conditions during her length of stay.
The skills and expertise of each person I came in contact during my mother's stay made me feel well assured that she was in the best care, and confirmed my decision to place her in the care of the Fairfield Rehabilitation Nursing Home was best.
Although my mother preceded her family in death just over a week ago, it would be unfair if the individuals whom provided her medical care during the last months of her life were not recognized. My mother was a fighter until she was called by the Lord to start her work in heaven. Please know if I were ever to recommend someone to a nursing home, the Fairfield Rehabilitation Nursing Home would serve as my number one recommended service provider.

We want to take this opportunity to express thanks and appreciation from the family of my sister, Elaine, (recently) discharged from your facility. We were so pleased with the professional and caring concern displayed by all of your staff, both clinical and non-clinical. The Nursing staff was always attentive to questions. They promptly responded and followed-up on any thing we asked. This was especially helpful in sorting out her course, especially with physicians. The hands-on care was immediate and most often anticipated any need. The CNAs were sweet, caring and competent and Elaine loved them.
Elaine's clinical care to recover from hospitalization for pneumonia was built around well-planned and executed physical rehabilitation that also included important activities to stimulate thought and memory. She loved her therapy and, therefore, quickly benefited from the activities. Family was always included in and informed about what she was doing. Your Rehab staff added a personal feel and special attention that made it much more effective and successful.
Very special gratitude goes to Roxanne who was absolutely invaluable to understanding and navigating the whole process from admission to the very important discharge process to an appropriate care level. Special thanks to Summer, who with her Rehab staff was key in Elaine's recovery. Also, special thanks to all the loving CNAs who cared for Elaine (Kulwanda, Randy and all).
